Registration

Registration is open for all 2024 - 2025 high school and middle school athletics.

Process

The registration form only needs to be completed once per school year, regardless of the number of sports a student participates in. 

To register on a computer:

  • Log in to the parent/guardian PowerSchool account
  • Select "Forms" in the menu on the left side of the page
  • Select the "24-25 Athletic Registration" form
  • Complete all required fields, including payment information

To register on the PowerSchool app:

  • Select Forms
  • Select the "24-25 Athletic Registration" form
  • Complete all required fields, including payment information

If you have any issues accessing Forms on the PowerSchool app, try uninstalling the app and reinstalling to the most current version.

Cost

The Middle School athletic fee will be $100 for the 2024 - 2025 school year
The High School athletic fee will be $175 for the 2024 - 2025 school year
Financial assistance is available. Scholarship information is available in PowerSchool with the Athletics Registration form.

The athletic fee is due once per school year, regardless of the number of sports a student participates in. 

Physicals

A copy of a student’s most recent physical must be provided in order for them to participate. There is no form necessary for the physician to fill out. A post-physical summary is sufficient, as long as it has the date of the exam and a note saying that the child is cleared for participation in school sports/activities. Here is an example of what is needed. 

FHS: Physicals are valid for two years from the date of the exam. High school student’s physical documentation should be sent via email to Rob Sullivan, Asst. AD and Beth Luce, FHS Nurse

FMS: Middle school student’s proof of physical documentation should be sent via email to Maureen Sylvia, FMS Athletic Director. Documentation may also be faxed from the doctor's office to 207-781-1019. 

Transportation Form

Due to the bus driver shortage, there are times when a bus is not available and private vehicles may be needed to transport teams to events. Please complete the online transportation form to indicate your preference for if a student can/cannot ride in a private vehicle. To complete the transportation form:
